Only school districts and Head Start agencies can apply for approval to operate pre-elementary schools.
This application is administered in compliance with the Alaska Department of Education & Early Development Regulations, Chapter 60, 4AAC 60.036. The application must be submitted by the school district or Head Start agency seeking to operate a pre-elementary school at least 30 days before the student start-date of the school. Annually, not later than September 15, an approved pre-elementary school must submit an application for renewal of department approval with a completed assurances form.
If your organization oversees any sites in addition to its main office, enter the location, number of hours children are served, and whether the site is also child care licensed (yes or no) for each site in the field below. If correspondence is to be directed to individual sites, provide contact information for each site as well.
Information for each site should be on its own line and separated by semi-colons.
