1. Log into https://ak.cnpus.com/cnp/Login and complete the sponsor application and site applications.
• • Update contact names and information. (two different people)
• • Update E-mail address and telephone numbers
• • Go to the “Checklist Tab” and complete all off-line forms needed for your sponsor type
2. Optional Documents to Return to CNP
• • CNP Web User Authorization Login Request (word)
• • CNP Web User De-Authorization Form (word)
• • PrimeroEdge Authorization New User Form (word)
• • PrimeroEdge User Deactivation Form (word)

When all online and offline documents are complete and approved by CNP, claims may be submitted for reimbursement in the CNP Web.
